Do you live in the US? Your description is certainly the case at most baseball stadiums. But in American football, mostly at college level (but pro as well), fans create an incredible amount of noise to try and disrupt the quarterback from relaying the play to the rest of his team (yes... without prompts). Teams take time outs to cope with the noise. Basketball is the noisiest sport I've ever been to, constant scoring + indoor arena = loud.
As for MLS atmospheres, I can say I've seen some good stuff from the DC United fans here. I think dcred (if he's still around?) could say more, but there is certainly some decent noise from sections of the crowd. I remember the disturbing 3 minute chant of "fuck you Beckham" at the LA match, as well as a massive banner reading, "Shouldn't you be in Austria?" I'll have to make it to a Toronto match next time I'm north.
