The world in general would say Madrid-Barça, cause even people that don't like football very much watch that game. I say in terms of greatness both are great, but Barça-Madrid calls more the attention because people rate it as THE epic match. And this year is even bigger with all Kaká, Ronaldo, Zlatan, Messi, Xavi, Xabi, Iniesta, Benzema, Higuain, etc. involved.
I honestly prefer United vs pool, as many followers of the Premier League do, and especially for us United supporters.
Boca vs River is for me up there in the top derbys.
