I've been seeing a few issues with the notifications on caf and not sure if it's the same for other users as well.
Basically the alert I get is "xyz replied to the thread abc" (a few seconds ago). The issue is that xyz had replied long back (maybe an hour or so) and not only have I seen the reply, but I might have even commented on the thread. It's not an issue in all the alerts, as in, in some of the alerts there has actually been new activity.
Let me know in case the issue is from my side and if there is any way I can resolve it from my end only.
