What affects how good a player is? Just rating, morale and fitness?
Real life performance effects the rating.
As for morale and fitness I'm not sure how much that effects the players performance in the game. @Damien and @green_smiley probably know more about this.
Rating.
As far as fitness goes, don't play anyone below 90%, ideally you should have everyone at 95 or thereabouts which is easy to do.
Morale isn't that big a deal.
All of that said, you may find some players performing better than their ratings suggest, just happens at times.

How many games do you have to field a player to remove concerns?
Karius is screwed. Level 4 concern and keepers expect to play 90% of games. If you want to keep him, you'd have to play him in all games to have a chance of it not going to level 5 and even then it isn't guaranteed.
