They can probably just arrest whoever's still paying for anything with cash in whatever year this is and close the case.

Inside job for sure. Not necessarily done by someone who works there, but probably someone they know such that they surmised how to remove all security/access protocols to gain entry into the facility for a short period of time.

Could have been an ex-employee with knowledge and potentially even access. The number of firms that are terrible at removing access for departed staff would make your head spin.

Could've very well been. These facilities usually have redundant, overlapping security such that if any one of them fails, the others would still provide sufficient security. For all human and electronic security to be neutralized so effectively would require very detailed knowledge of all security protocols.
