The use of wide areas is our main culprit.
We absolutely suck at crossing and getting on the end of them in general. Rarely do we score tap ins from 5 yards, and we hardly even get in behind teams at the best of times.
Most of our goals from from quick interchanges in the centre of the pitch rather than somebody doing something individually out wide.
