Precovid there were more people in Itaewon during Halloween AND some crowd controls measures were taken even if there was no one organizing it. It isn't a new spontaneous thing, people were aware of past Halloweens at Itaewon and assumed the 2022 one would have the same level of security. But new mayor, new government, the police was understaffed and even asked some private assistance that was ignored.
Anyway, I learnt the security and safety rules (concerning anything) aren't the ones I expected. I wrongly assumed they would be the same than Japan or EU. So some previous things I was baffled about made more sense knowing that. Like what kind of city is fine offering a bare place without facilities or anything as a concert ground for 100k people? Or a previous known accident.
