I like Kei so I hope this is a 'real' injury. Didn't look injured when he won the fourth set..
He does have the most fragile body on tour to be fair.

Same here, but he did receive coaching from Gilbert, so some strategy is surely involved. He did look miserable on the ground, though.

Haven't seen him serve this well in a fifth set in years. The back problems seem to be banished.

Incredible achievement for a player his age. Apart from a horrible start and one terrible service game in the fourth he played a lot of oustanding tennis against an absolute 5 set specialist (Federer now 26:5, Nishikori 15:6).

When was the last time Federer played this well in a fifth set? I really can't think of any match in the recent years.

To be fair, he was playing peak Novak in these 5 setters.
