Based on reality, from what I understood they have a team of people who vote to update the rating based on real life performances and they update it from time to time. This way the game is always truthful to reality i.e. the best players in ral-life will be the best players in the game.
In the game you can see what's being updated, right now they're updating Switzerland and Poland teams.
Muniain to Bolton and Javi martinez to Newcastle, good deals, these players will surely improve in the next update.
